Introduction and referral form

St Nicholas Hospice Care is here to help people live their lives to the full and improve the quality of life for all who are affected by their illness.

We see people with any illness that is life threatening, such as Cancer, Multiple Sclerosis, Motor Neurone Disease and other advanced diseases such as heart or lung diseases. For details on how to refer click here. We can:

  • share information and give advice on all aspects of care, from diagnosis through to advanced disease Dr Kitchen with a patient
  • offer emotional support to patients, families and carers
  • help people maximise their quality of life through rehabilitation
  • advise on relief of pain and other symptoms
  • liaise and act as advocate with other services which may be able to help
  • offer financial advice about benefits and grants

Care is delivered by a dedicated skilled multidisciplinary team, supported by trained volunteers and an accomplished administrative team. We welcome feedback about our service, please either write to the Clinical Services Director or complete the online feedback form.

We always work closely with GPs and District Nurses to support people in their own homes and our team includes:
